This position will allow the right candidate to build lasting relationships with patients, while providing 1:1 care.
Responsibilities of the home care nurse include, but are not limited to the following:
- Assist with patient's ADL's and IADL's, such as bathing, grooming, meal prep, light housekeeping.
- Monitor patient’s condition, reporting any change in status to the Supervising clinician.
- Electronic charting using Axxess.
- Foster a supportive and compassionate environment to care for patients and assist their families.
- Expand knowledge and capabilities by completing assigned In-Services.
